If your WIC benefits (vouchers) have been lost or stolen, call the WIC office at 952-891-7525. If the WIC rules allow for the benefits to be replaced, staff will ask you to come to the WIC clinic.
If staff asks you to come to the WIC clinic, please complete these forms and bring them with you. The forms are in English and Spanish.
WIC benefits can be replaced only once in a one-year period for a WIC household.
