Refugee Health Assessment
Dakota County Public Health Department staff work with private health care providers to offer new refugees a comprehensive screening examination and follow-up or referral.  Refugee health assessments helps diagnose health problems and helps protect the health of others in Minnesota by treating communicable diseases. Data from the assessments assists the Minnesota Department of Health to predict trends in diseases, set policy, and assess needs for services.

Green Card Vaccination Form for Refugees
Refugees who need to submit form I-693 to the U.S. Citizenship and Immigration Services (USCIS) can make an appointment with the Dakota County Public Health Department.

There are certain requirements:
  • You must be a refugee who is living in Dakota County.
  • You must have lived in the United States at least 9 months.
  • Your vaccinations must be up-to-date. If they are not up-to-date, get the vaccinations updated before scheduling an appointment. If you have health insurance, get the vaccinations at your clinic. If you do not have health insurance or do not have a clinic, you can get vaccinations at the Dakota County Public Health Department – there may be a fee for the vaccinations.
  • You must pay for this service. The current fee is $81.00 per person.

It can take up to 4 weeks to process the paperwork, so if you need your I-693 form processed more quickly than that, contact another Civil Surgeon.
