You can establish paternity by filing a Recognition of Parentage form with the Minnesota Registrar of Vital Statistics. The form must be signed by both the mother and biological father and notarized. It can be signed at the hospital at the time of the child’s birth or after the mother and child leave the hospital.
There is no cost to file a Recognition of Parentage form.
Get a Recognition of Parentage form
You can receive a Recognition of Parentage form by doing the following:
Does not grant visitation or custody
Signing a Recognition of Parentage form does not give the non-custodial parent parenting time (visitation) or custody rights until the court issues a court order granting these rights. It gives the non-custodial parent the right to go to the court and ask for parenting time.
Does not establish child support payments
Signing a Recognition of Parentage form does not require the non-custodial parent to make child support payments until the court issues a child support order. It gives the custodial parent or the Child Support Agency the right to go to the court and ask for support from the other parent.
