A provider’s first license is a one-year license. When you have been licensed for nine months, you will receive a packet of forms in the mail to start the relicensing process. In the packet, you will receive the following forms:
- Application
- Bureau of Criminal Apprehension Forms
- Home Safety Checklist
- For Your Visit Form
- Privacy Right Form
- A letter stating the month of your home visit
You will need to complete these forms and submit them to the agency along with a fee of $200 for a two-year license. Submit the forms by mail to Dakota County Social Services, 14955 Galaxie Ave., Apple Valley, MN 55124
Your worker will make a drop-in visit to your home during the month mentioned in the letter with your relicensing packet. We do not tell you the actual date or time; however, if there is a time that does not work for you, we ask that you let us know.
At the visit, we will go through a short interview with you, check the paperwork you have on all the children in care, and make a thorough inspection of your home to see that it meets the rules. We will answer any questions you have. If you need any resources, you may enter these on the For Your Visit Form, and we will try and accommodate your request.
