The following forms are for Dakota County currently licensed child care providers to complete and return to Dakota County to process for relicensing visits. Forms should be completed 3 months prior to expiration of the child care license.
Complete and return the following forms to Dakota County Social Services, 14955 Galaxie Ave., Apple Valley, MN 55124. The forms can be downloaded by clicking on the attachments at the end of this information:
- Child Care Application
- Child Care Checklist
- Enrollment Form
- Background Study Form (For each person over age 13)
- For Your Visit Form
- Privacy Rights Form (keep one copy for self and return 2nd copy to Dakota County)
- Training Record (copy showing your training and all other caregiver's and helper's training)
The following forms need to be available to show your licensing worker at the time of the visit along with all forms on the children in your care.
- Escape Plan
- Provider Policies (your own or our form completed and shared with all clients)
- Child Care Notification Form (have a copy with each child's records signed by parent)
